Your Records, Privacy, & HIPAA

Hope’s Door New Beginning Center (HDNBC) maintains a complete electronic record of all solutions provided. These records are protected by state and federal laws governing privacy rights of victims. They are released only with the informed consent of the individual who received help from our organization, unless provided otherwise by law.

Legal Disclosure

We may access and/or disclose personally identifiable information if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law or (b) comply with legal process served on us.

Mandatory Reporting 

HDNBC staff, as all adults in Texas, are considered mandatory reporters by law. In situations regarding the welfare of a minor, any disclosed information may be reported to protective solutions as required by law. 

Getting Copies of Your Records

If you need a copy of your record, or your child’s record, you may contact the receptionist at HDNBC who can assist you with filling out a Records Request Form. We do require a fee paid up front before the record will be released. Because HDNBC provides free solutions, this fee is to help offset the costs of maintaining and producing any records. If you are still receiving help from us, you may contact the primary staff person assigned to assist you. Once a form is received, it is forwarded to our Custodian of Records who will produce the records within 10–14 business days. The Custodian will contact you directly with any issues concerning your request.
